NEED A WORK PERMIT?

STEPS TO COMPLETE WORK PERMIT PROCESS:

1. Download & print work permit application below or pick up one in room 113

2. Student, Employer & Parent sections must be completed and signed

3.  Submit completed application to swomack@lbschools.net or in person in room 801

4.Process can take 1-2 days to complete so please plan accordingly. Pick up completed applications in room 801.
